Seventy pages, one to a shift. Each carries four fields, date, shift, truck, hour meter, then eight checks: tyres and wheels, forks and mast, chains and hoses, horn lights and beacon, brakes and steering, seat belt, battery or fuel, leaks under the truck.
Under the checks sits a boxed space for defects found, and who was told, three lines of it, then two signature lines: operator, supervisor. A check nobody signed is a check that did not happen, on paper or off it.
The book does not tell you what counts as a defect or when a truck comes out of service. That call belongs to your site's safe operating procedure and the manufacturer's own inspection sheet, and it differs machine to machine. The eight checks give you somewhere to record what you found, not a standard to measure it against.
