Sixty pages, five columns: date, load, carrier, destination, ticket no. A load with no ticket number against it is a load nobody can trace back to the paperwork that is supposed to prove where it went.

Twenty-nine college-ruled pages sit at the back, twenty-four lines apiece, for why a carrier was turned away, what a site refused to take, and who queried a ticket after the fact.

It does not print your carrier's registration requirements or which loads need a consignment note. Those come from the waste regulation you operate under, and they differ by jurisdiction and by what is actually in the load. Record the transfer in the five columns; the rule that governs it is not printed here.
