Twelve checklist pages open the book, ten points to a page: guards in place, leaks, noise, vibration, temperature, lubrication, fasteners, isolation, housekeeping, labels. The same ten questions, asked the same way, is what turns a walk-round into a record somebody can read back.
Twenty-four day-book pages follow, ruled into four columns: Date, Asset, Finding, Action. A finding written once, against the asset it belongs to, is a finding the next shift can act on instead of repeating.
Twelve pages of notes close it out, college ruled, for whatever a checkbox cannot hold. It does not set your inspection interval or your pass or fail criteria. Those are decided by the equipment owner or the rule that applies to the site, and both vary. The checklist gives you the columns; the numbers stay yours.
