Billable time goes missing in the gaps between appointments. Fifteen minutes on the phone before a hearing, an email answered from a taxi, a call that ran long because the other side would not settle: none of it sits in a calendar, and by the time you are back at your desk it has already become a guess. A guess is not what goes on an invoice.
Every week gets one ruled page, split into five fields: start, stop, client, matter, and whether the entry has been sent to billing. The same five fields, week after week, so a file reopened months later reads the way it did the day you wrote it. Behind the weekly run, lined notes pages catch whatever a five-field row will not hold: a conflict check, a retainer renegotiated mid matter, an instruction that changed twice in one phone call.
It sets no rate and no rounding convention. What you charge for the hour, and whether you bill in six-minute units or round to the quarter, are decisions your firm makes, and they differ by firm and by jurisdiction. A printed book cannot be corrected once the year is underway, so it leaves both to you.

- Pen test leaf
- The last page, printed for it on purpose. Every notebook is used this way; printing it deliberately is cheaper than losing the last leaf to it by accident.
