A handover is the one moment in a shift where a fact can vanish without anyone noticing. The outgoing operator knows something the incoming one does not. If it does not get written down in the two minutes before the gate, it stays lost until it becomes a problem on someone else's watch.

Each shift gets its own block: the state of the plant as the shift ends, and what is still outstanding for the next one. A line for off and a line for on sits underneath, naming who is answering for the plant at each end of the shift.

Three shifts fit on one page, so a full day of handovers sits together rather than scattered across three separate slips that someone has to chase down later. The book does not decide what counts as outstanding. Whoever is walking off the floor decides that, every time, and writes it down before they leave.
