Four pages at the front are ruled into two columns, headed Page and Subject. Nothing else in the block fills those columns in for you: every dot-ruled page that follows carries a printed number in the corner, and the index only works if that number gets copied across as each page fills.
Dots sit 5.0 mm apart, thirty-four rows to the page, drawn as geometry rather than a tiled pattern, so the lattice holds its edge under a close zoom instead of resolving into a scatter of soft squares.
The index page does not fill itself in. Skip it for the first thirty entries and the number in the corner is the only thing telling two dot pages apart.
