A meeting produces two different kinds of text, and most notes blur them together. What was discussed is a record of the conversation: long, useful for context, and not itself a decision. What was decided is shorter and belongs on its own line, because six months later somebody will ask what the meeting actually resolved, not what was said along the way.
Each meeting opens with who was there and who sent apologies, then moves through discussion, decision and action in three separate blocks. The action block is a small table: what has to happen, who owns it, and by when, so a resolution does not quietly stop being anyone's job once the room empties.
It sets no procedural rule. Standing orders, quorum, and who may second a motion differ by organisation, and a printed book cannot be corrected once a rule changes. It holds what was decided and what happens next.
