Sixty pages, five columns: date, asset tag, change made, how to roll it back, by. A change with no rollback written down is a change nobody can undo at two in the morning, when it turns out to be the wrong one.
Twenty-nine pages of notes trail the log, college ruling, 24 rows deep, and they hold what a column cannot: who approved the change, what the vendor's advisory actually said, why a rollback was needed after all.
It does not print your change approval threshold or your change window. Those come from your organization's own change control policy, and they differ from one IT department to the next. Record what changed and how to undo it in the five columns; the policy that governs when you may stays with your organization.

Is it dated?
No. Start it the day the asset register does.
Does it print my change approval threshold or change window?
No, and that is deliberate. Those come from your organization's own change control policy, and they differ department to department. The columns give you a place to record the change, not the policy behind it.
What ruling are the notes pages?
College, 7.14 mm, twenty-four lines to an A5 page.
Why does every row ask for a rollback?
Because writing down how to undo a change before you make it is cheaper than working it out afterward, under pressure, at the wrong hour.
All registers which, it is acknowledged, ought to be kept secret, ought certainly never to exist.
