A fuel receipt left loose in the glove box survives about as long as the drive home. This log fixes four columns for it instead: date, the odometer reading, what was done, and what it cost. Write the reading down at the pump and the receipt itself can go in the bin.
The same four columns appear twice over: across sixty rows for the plain run of the year, and again across eight pages that group them by month, so a March fill-up turns up without hunting through sixty separate days to find it. A single notes page and a year page close out the rest.
No mileage rate and no allowance appear anywhere in it. A tax authority sets that figure and revises it most years, so the log holds the reading rather than the rate. What the distance is worth gets tallied later, against whatever number the tax office publishes that year.

What are the four columns?

Does it calculate mileage claims for me?
No. It holds the odometer reading; the rate that turns distance into a claim is set by a tax authority and changes most years, so it is left for you to apply.
Is a mileage rate or allowance printed inside?
No, for the same reason. A printed rate goes wrong the day it is revised, and it cannot be corrected on paper.
Is it dated?
Yes, for one calendar year, front to back.
