A patient gives a name at the desk, says what is wrong in a few words, and settles the fee before leaving. Four columns hold that exchange: a running number, the patient, the complaint, and the charge. None of them asks for more than a line, so the desk keeps moving to the next name.

The complaint column takes a phrase in the patient's own words. 'Sore throat' or 'follow-up' sits there without trouble, and in a bad flu season the same word turns up in that column dozens of times with nothing lost by the repetition. A diagnosis code would not sit there the same way, because a printed page cannot be locked, redacted or amended the way a proper clinical file can.

It prints no fee schedule and no rule for how long a finished book should sit in a drawer. What a visit costs is set by the practice, not by a notebook, and how long the record stays is a question for whichever authority governs where the practice sits. The book gives both numbers somewhere to be written, once you already know them.
